SharePoint Online doesn't convert a Word document into a browser based form. A Word file can contain fillable fields, but it will still open as a document rather than a web form. Option 1: Keep your document as a fillable Word form

Go to https://forms.office.com/ and create a new form.
Add questions that match the content of your Word document.
