    Thank you for posting your question to Microsoft Q&A Forum.
    As of now, there hasn't been a direct “show explanation box when Yes is clicked” feature yet but you can use branching logic in Microsoft Forms as a workaround.

    To add an explanation section when “Yes” is selected:

    • First you have to create your initial question.

    Example: “Do you agree with the policy?” with options Yes and No.

    • Add a follow-up question for explanation

    Example: “Please explain why you selected Yes.”

    Set this question type to Text (long answer).

    • Then you can enable branching

    In Microsoft Forms, click More form settings (⋯) → Branching.

    If Yes, branch to the explanation question.

    If No, branch to the next relevant question or End of form.

    Save and test

    When a respondent clicks Yes, they’ll be taken to the explanation question. If they click No, they skip it.

    Note:

    • Branching works on choice questions only, so your first question must be multiple choice.
    • You can add multiple branches for different answers if needed.
    • This feature is available in Microsoft Forms for personal, business, and education accounts.
    • You can't do this workaround from the Poll feature in Outlook; this has to be done from Microsoft Forms.
